Members of the Air Traffic Evaluations and Investigations Staff, AAT-20, conducted a followup evaluation (FUE) for YYY via desk audit on May 1, 2000. The evaluation consisted of a review of the previous full-facility evaluation (FFE) report, the ATM's responses for items that were rated as "A4-5A," "M," or "P" during the previous FFE, and the hub/region's endorsements. In addition, evaluators reviewed data that was requested for desk audit. The ATM was telephonically briefed on May 2, 2000.
The ATM's next response for open items, along with the hub/region's endorsements, is due at AAT-1XX no later than July 2, 2000.
1. OPEN ITEMS.
NOTE-
These items were closed during the response process, but re-opened during this evaluation.
(a) Operations. Two items were identified.
(1) (99-T-YYY-01M) SOP DIRECTIVE. The SOP directive had not been corrected to include jurisdictional boundaries for the Arrival South position. In addition, the airport diagram within the SOP directive had yet to be updated to reflect new taxiways (7210.3, par. 2-1-2).
NOTE-
The SOP directive was rated as a problem during the FFE conducted in June 1999.
(2) (99-T-YYY-02M) PHRASEOLOGY. Evaluators reviewed ## in-flight evaluation reports that were conducted since the FFE and a 2-hour tape recording of the Arrival South position that was submitted for desk audit. Specialists continued to employ nonstandard folksy phraseology. In addition, a review of the ATM's responses revealed that the required three-step problem-closure process was not accomplished and documented (7110.65, pars. 2-1-17, 2-4-17, 2-4-19, 2-4-20, and 4-5-7).
NOTE-
Phraseology was rated as a problem during the FFE conducted in June 1999.
(b) Operational Support. One item was identified.
(99-T-YYY-05P) TECHNICAL TRAINING RESPONSIBILITIES. The ATM was unable to submit documentation for desk audit to substantiate that the required annual review of technical training had been conducted (7210.56, par. 3-1-2).
2. NEW ITEM.
Operations. One item was identified.
LAHSO PROCEDURES. The ATM advised that LAHSO was temporarily suspended pending upgraded airport lighting, new signage, and revision of the LOA with the airport authority.
3. CLOSED ITEMS.
(a) Operations. Two items were identified.
(1) (99-T-YYY-03P) MSAW ALARM CHECKS. The equipment checklist for each watch was updated to include MSAW/CA alarm checks.
(2) (99-T-YYY-04P) CERTIFICATION SKILL CHECKS. A review of FAA Forms 3120-25 that documented certification skill checks since the FFE revealed that times were entered in block 8 and a recommendation for certification was checked in block 13.
(b) Operational Support. One item was identified.
(99-T-YYY-06M) LETTER TO AIRMEN. The required letter to airmen regarding non visible movement areas was published.
